How to Refer

All referrals for Cancer Screening related colonoscopy are to be submitted to Central Intake rather than directly to the specialist
If the referral is sent directly to the specialist, the respective office will have to route the request to Central Intake, resulting in potentially delayed care

Mandatory Requirments for Ocean & eFax Referrals

Referral forms MUST be submitted with the following:

Responses to all mandatory fields
A copy of the Cumulative Patient Profile (CPP)
Previous colonoscopy operative & pathology reports
Abnormal FIT lab results

Ocean e-Referral

Ocean eReferral is the preferred modality for referrals as it allows providers a simple and secure platform to send referrals, while providing progress updates regarding the patient’s care. To send eReferrals, please access our HealthMap Listing.

Ocean eReferral can be installed directly into existing office EMRs for users of Telus PPS, Accuro and certain versions of OSCAR. This eReferral solution comes at no cost to referring primary care providers (PCPs) and our team can assist with the implementation and training.

Ocean Referral will allow the PCP to send referrals directly from the patient's chart. The referral form will auto-populate with the patient's information and a copy of the referral will automatically be added to the chart.
